Click here for a printable PDF version About the Tax Credit Program The Tax Credit program allows every person who files an income tax return with the State of Arizona the opportunity to receive a tax credit for contributing to extracurricular activities. These tax credits support extracurricular and character education programs in the Madison School District. Anyone can participate, married or single, with or without children. Married couples filing jointly may contribute any amount up to $400. Single persons, heads of households, and married couples filing separately may all contribute up to $200 . Donations may be used for any school sponsored activities that require students to pay a fee in order to participate or for a character education program. Examples include: foreign language, field trips, enrichment activities, athletic and fine art programs, and after-school clubs.
How the Program Works For every dollar you donate, the amount you owe on your 2010 Arizona taxes will be that much less. If you do not owe taxes, the amount you donate may be carried forward for up to five years to offset future tax liabilities. For example, if a single person owes $300 on their state taxes for 2010 and donates the maximum amount of $200, they will then only owe the state $100. If a single person owes $100 on their state taxes for 2010 and donates the maximum amount of $200, they will owe nothing on their taxes for this year and can still use the remaining $100 to offset future taxes.
How to Donate Donations may be made online through our secure website, in person, or by mail. To donate online: click on the button above or below to make a donation using our secure website. Please note, the best time to donate online is after school hours for the fastest transaction. To donate in person: You may bring the form below and your donation to the Madison District Office in person at 5601 N. 16th Street, Phoenix, AZ 85016. Hand-delivered contributions must be received by December 17, 2010. To donate by mail: print the form below and return along with your check to the school of your choice or: Madison Tax Credit Program Attn: Jennifer DeFonce 5601 N. 16th Street Phoenix , AZ 85016 Mailed contributions must be postmarked no later than December 31, 2010. You will receive a tax credit receipt at the time you make your donation if it is hand-delivered, or by January 31, 2011 if it is mailed. Be sure to keep your receipt(s) for tax purposes.
