2020-2021 Registration Days

  • Welcome Madison Families! Please complete the 3 steps below to register your child for the 2020-2021 school year. This process is only for families who have a continuing Madison student, those approved through Open Enrollment and new Madison in-district families. If you have not been offered a seat via Open Enrollment or have not enrolled your in-district student, please complete those steps first.


    Registration Days will be August 4, 5 and 6. You will complete your child's registration on the day listed below based on:

    • August 4- New Students to Madison, Kindergarten, and 5th Grade Registration
    • August 5- Returning Student Registration Last Name "A" through "L"
    • August 6 – Returning Student Registration Last Name "M" through "Z"

    Step 1: Please complete the Student Safety Form

    Our highest priority is the safety of our students. Each parent is expected to complete the Student Safety Update (formerly the Emergency Contacts/Yearly Update) Form so that we have the MOST recent contact information for your family for communication purposes.

    Step 2: On-Site Online Learning Sign-Up Form

    The on-site online learning option, per the Governor’s Executive Order, will be available to meet the needs of students who qualify for Free & Reduced Lunch, or students who receive Special Education Services, or students who are English Language Learners (ELL), or students in the care of the Department of Child Safety. While the on-site online learning option is intended to meet the needs of the students listed above, the Executive Order requires that this accommodation be available to every student. Students will have the option to complete their online learning at a physical location on the date the Governor reopens school, currently August 17.

    If your student plans to learn online at home for first quarter, you do not have to complete this form.

    Please use the link below to sign up for the on-site, online option only if needed. Please complete this form for each student participating.

    Step 3: Complete the Free & Reduced Meal Application

    We ask all families to complete the Free & Reduced Meal Application form so that we can make sure that your child is receiving the appropriate food service. Take a look at some of our menus!

    In order to practice social distancing and to keep our students, parents and staff safe, we have made the decision to implement a cashless meal service. We will no longer be accepting cash payments during the breakfast and lunch meal service.

    Academic Benchmark Assessment Information:

    The Governor’s Executive Orders outlines a plan for the safe reopening of Arizona schools. Part of the plan requires all school districts to conduct academic benchmark assessments within the first six weeks of the 20-21 school year for all students.
