Birthday Program

Madison offers a “Birthday in a Box” program for your child. We make it simple for you to order a classroom celebration for your child and provide a range of options to meet your needs.  We make it simple for you to order a classroom celebration treat for your child and provide a range of options to meet your needs. All of these treat options are USDA Smart Snack Compliant and are compliant with the Arizona Healthy Schools Act.  Birthday treats do not contain tree nuts/ peanuts in ingredients, but MAY be manufactured in a plant that contains them.  This form can be completed at any time, but must be completed 10 days prior to requested date and paid in full 7 days prior to requested date
