English 2024-2025 Open Enrollment Interest Form
Spanish 2024-2025 Open Enrollment Interest Form
Open enrollment is for families who live outside the district or school boundary area. Families must complete their online Open Enrollment Interest Form and will be given priority according to the groups the district has established. All Open Enrollment Forms received between December 1, 2023 and January 26, 2024 will be eligible for the priority group classification, per district policy.
MTA is lottery based. All applications received between December 1, 2023 and January 26, 2024 will be eligible for the lottery based on priority group classification, per district policy.
Forms received after that date will be placed according to the date and time the form is received. Please see our Open Enrollment Procedures page for more information on open enrollment and the priority groups.
When beginning the Open Enrollment Interest Form, you will be asked to select your Student Type:
-
Non-resident Student is a student who resides outside of the District's boundaries who seeks to enroll in the District.
-
Resident transfer Student is a student who resides in the District who seeks to enroll in a school outside of the student's attendance area.
-
Currently Enrolled Student is a student seeking transfer to another school in the District.
Current Open Enrollment Students
Families of current students do not need to complete an Open Enrollment Interest Form unless you are seeking to transfer to another Madison campus.
If a student will be attending the feeder middle school associated with their current elementary school next year, no Open Enrollment Interest Form will be needed. If you are interested in attending a different school other than the feeder middle school, the 2024-2025 Open Enrollment Interest Form must be submitted.Click here to view Madison's District and school boundary map.
